Trasylol: Increased risks of heart attacks and strokes A recent study was conducted to determine whether Trasylol was better than either the generic tranexamic acid or aminocaproic acid. In this study, patients who were administered Trasylol had a higher rate of death than the other two agents. Because of the increased risk of heart attacks from Trasylol – this important study or trial which was reported in the New England Journal of Medicine ended earlier than planned. In clinical trials, when compared to a placebo, Trasylol and the two other acid agents proved to be more effective in reducing the need for blood transfusion. As for costs, the prices vary wildly. With Bayer’s Trasylol, a four-hour heart procedure costs $1,400 while the aminocaproic acid costs a mere $4. You have to wonder if all things are equal: Why would a hospital opt to pay $1,400 instead of $4 and pass the savings along to the patient’s healthcare provider or directly to the hospital? There is a concern that Trasylol has increased the risk of heart attacks and have increased the risk for strokes as well as kidney failure and short and long term mortality. The controlled study was called BART - Blood Conservation Using Antifibrinolytics in a Randomized Trial (BART). The study was approved by the research ethics committee at each participating center. Written informed consent was obtained from all patients. None of the pharmaceutical companies making the study drugs contributed medications or financial support nor in the designing the study. All the patients underwent high-risk cardiac surgical procedures and were randomly chosen as to who received the Trasylol and the other two agents. Causes of death were grouped post hoc as heart-related or non-heart-related. Cardiac causes consisted of congestive heart failure, shock, myocardial infarction, and right ventricular failure. Non-heart-related causes included hemorrhage, stroke, sepsis or multi-organ failure, and other or unknown causes. A total of 108 of 2,331 patients died within 30 days after study randomization. The 30-day rate of death from any cause was 6 percent in the Trasylol group; as compared with 3.9 percent in the tranexamic acid group; and 4.0 percent in the aminocaproic acid group. The study was terminated on October 2007 on the recommendation of the independent data and safety committee who advised termination because of a strong trend toward higher mortality in the Trasylol group than in the other two groups.
