What product was recalled?
ReliOn Insulin Syringes-31 gauge 1 cc (100 syringes per box); lot number-813900; product identification number-38396-0403-02.

What does the syringe consist of?
ReliOn Insulin Syringes consist of a syringe barrel, a plunger rod, and a hypodermic needle attached to the tip of the syringe.

Where were the syringes recalled from?
Wal-Mart and Sam’s Clubs stores; these are the only places that sell this type of insulin syringe.

How many syringes were recalled?
4,710 boxes that hold 100 syringes each for a total of 471,000 syringes.

What does a gauge number mean?
The higher the gauge number, the thinner the needle. Gauge numbers for insulin syringes are 27 to 31.

How can a mix-up like this happen?
Some syringes labeled for use with U-40 insulin got packaged with syringes for U-100 insulin.

What does U-100 mean?
U-100 stands for the insulin concentration/strength. Most insulin syringes sold in the United States are U-100 and made for U-100 insulin. U-100 insulin is common in the U.S. and Canada. U-40 insulin is more common in Europe and has a lower concentration.

How were customers notified?
On October 9, 2008-Tyco recalled the lot voluntarily asking that any units of the affected product be removed from inventory and placed in quarantine. The recall was posted in stores and posted on the website. Wal-Mart sent letters to more than 16,500 customers notifying them of the recall.

When were the syringes sold?
Wal-Mart sold the syringes at Wal-Mart stores and Sam's Clubs from Aug. 1, 2008, until Oct. 8, 2008.

What are the consequences for taking the wrong amount of insulin?
According to the FDA, patients who receive an overdose of as much as 2.5 times the intended dose may lead to hypoglycemia, serious health consequences, and even death.
